Neuroaffirming practice in paediatric occupational therapy recognises that neurological differences, such as autism, ADHD, and other neurodivergent conditions, are natural variations in how brains develop and process the world. Rather than trying to “fix” or normalise children, neuroaffirming therapists prioritise understanding and supporting each child’s unique way of thinking, sensing, communicating, and interacting with their environment. This approach is centred on respect, acceptance, and collaboration, helping children to develop skills without compromising their identity.
In practice, neuroaffirming occupational therapy looks very different from traditional deficit-focused approaches. Therapists work closely with children and their families to identify meaningful goals and create strategies that enhance participation in daily life, such as play, learning, self-care, and social engagement, while accommodating individual differences. For instance, a child who is easily overwhelmed by sensory input might be supported with sensory tools, scheduled breaks, or alternative environments that allow them to engage comfortably. Language and goal-setting are also crucial in neuroaffirming practice. Rather than labelling behaviours as “problems” or prioritising conformity, therapists focus on strengths, preferences, and abilities. Goals might include helping a child develop routines that support self-regulation, learn strategies to navigate school environments successfully, or participate in activities they enjoy without stress or pressure to mask their natural traits. This approach fosters self-esteem, resilience, and autonomy, rather than reinforcing anxiety or feelings of inadequacy.

Research and lived experiences of neurodivergent individuals highlight the negative impact of traditional therapy models that emphasise compliance or “normal” behaviour. Masking or suppressing one’s natural responses can increase stress, anxiety, and long-term mental health challenges.
